In Discover Series 4, we bring to you our fourth series of online educational talks, this time covering the ever popular subject of diet and fluid requirements for patients living with a stoma, as well as exploring the specific needs for patients who are experiencing a high output from their stoma. We will also explore the pharmaceutical management of a high output stoma during the final session of Discover 4.
Session 1: Exploring diet and fluid needs for ostomists
In this opening episode Melanie Baker, Clinical Lead for the Nutrition Support Team in Leicester, explores why dietary changes are sometimes necessary for people following stoma formation.
Melanie will also discuss some of the physiological changes to the gastrointestinal tract that can occur after surgery and the implications that it may have on patients. She also helps us to understand how the body absorbs certain electrolytes, and the impact that this can have when the GI tract is compromised.

Melanie Baker Biography
Melanie currently works in Leicester as the Senior Specialist Dietitian for the Nutrition Support Team, managing a caseload of complex nutrition support/ intestinal failure patients, including patients on Home Parenteral Nutrition. Her undergraduate training was completed at Surrey University in 1996 and she was awarded an MSc in Heath Service Research in 2005.
Melanie has been a tutor on the British Dietetic Association postgraduate nutrition support course and is the regional representative for BAPEN (British Association of Parenteral and Enteral Nutrition).
Research and publication interests include management of patients with high output stomas and complications of PN. Melanie is currently undertaking a randomised controlled study investigating the effect of home Jejunostomy tube feeding on quality of life and nutritional parameters in patients with Oesophago-gastric cancer after Oesophagectomy/ Total Gastrectomy. She has co-authored two book chapters (in “Home Parenteral Nutrition” by Bozzetti, Staun and Van Gossum and “Nursing and the Surgical Patient by Pudner)

Session 2: Hydration Education: getting the balance right for high output stomas
In this next episode we will hear from Melanie Baker, Clinical Lead for the Nutrition Support Team in Leicester. Melanie will be exploring the challenging issue of maintaining adequate fluid and electrolyte support for those patients experiencing a high stoma output. Melanie will remind us of where and how certain GI fluids are secreted and absorbed and how these impact on patients.
- To be able to identify when a patient is in high output and initial action to be undertaken
- To have an awareness of how having an ileostomy impacts on normal digestion and fluid and electrolyte absorption
- To increase your knowledge and understanding of the fluid and electrolyte needs of a patient experiencing a high output stoma

Session 3: Pharmaceutical considerations in the management of high output stomas
In this episode we will hear from Sarah Cripps, Consultant Gastro Pharmacist at Oxford University Hospitals NHS Trust. Sarah will guide us through some of the important pharmaceutical considerations, as well as the plethora of medications that can be used in the management of patients experiencing a high stomal output. Sarah will also highlight how and where certain drugs are absorbed and the impact this can have on patients.
- To identify some of the common consequences that can occur when a patient is experiencing a high output stoma
- To gain valuable insights and guidance to enhance your clinical practice in relation to drugs and drug therapies used in the specialist sphere of stoma care
- To have an awareness of the impact of specific drugs following a stoma formation, and especially those used in management of high output stomas
Sarah Cripps Biography
Sarah has worked at the Oxford University Hospitals NHS Foundation Trust since qualifying in 1995 from the University of Bath and registering with the GPhC. She has experience in a wide range of clinical specialties, pharmacy management and education and training. Her main role has been as a specialist pharmacist in Gastroenterology/hepatology for the last 23 years (10 years as a consultant pharmacist in this specialty).
Sarah is an independent pharmacy prescriber and Faculty Fellow of the Royal Pharmaceutical Society.
- Sarah is the current co-chair of the UKCPA Gastroenterology and Hepatology interest Group committee and treasurer of the British Viral Hepatitis Group (BVHG). She was previously the nominated pharmacist for NHSE Hepatobiliary and Pancreas CRG.

- She is a member of the BSG IBD Guidelines Development Group
Sarah teaches regularly at local and national conferences and is the author of several publications. Her main research interests include *Inflammatory Bowel Disease Patients’ beliefs about their Illness & medication and implications for medicines adherence and patient education, * Therapeutic drug monitoring of biologics in medicines optimisation and * Roles of pharmacy in hepatitis C elimination.
Sarah contributes to the development and update of medicine related policies for Gastroenterology within the OUH and Thames Valley
